- Applicant photo (passport size) of proprietor / partner / director / authorised signatory.
- Government‑issued photo ID such as Aadhaar, PAN, Voter ID or Driving Licence.
- PAN of the business entity or proprietor.
- Business constitution proof – Certificate of Incorporation, Partnership Deed, LLP Agreement, Shop & Establishment Certificate or similar registration.
- Proof of address of business premises – electricity bill, property tax receipt, rent agreement with NOC or similar.
- List of proprietors / partners / directors with contact and identity details.
- List of food products / categories to be handled under the FSSAI license.
- Food Safety Management System (FSMS) plan / declaration, and FSMS certificate for some central licenses.
- Layout / blueprint plan of the processing or manufacturing unit (if applicable).
- Water testing report for units where water is used in processing.
